DEAF BAPTIST FELLOWSHIP OF AMERICA: Registration Form

FREE REGISTRATION FEE FOR ANYONE!

Any person that brings 9 adults (18 years old or older) to the Deaf Baptist Fellowship of America at Scottsdale, Arizona on June 29 to July 3, 2009, registration fee ($110.00) is free. Registration fees must be sent with a money order to the address on the completed form to avoid the late fee. You are responsible for late fees if you wait until the last minute. You will be charged with a late fee if you have discovered that you do not have 9 adults confirmed. If you paid for your registration fee you will receive a refund on Friday, July 3, 2009 when your 9 adults are confirmed. Late fees are not refundable.
